Subject: Leaving for personal medical issues

To whom it may concern,

I hope this message finds you well. I am writing this email to inform you about my current medical condition which requires several days of treatment.

After consulting with my doctor, he recommended me to stay at home more than 5 days to be completely recovered from this tough disease, and not to spread it among the rest of the students. This seems convenience to me, as newly released medicines are more widely available in my neighborhood compared to the street with the institution.

I was wondering If you could contact with my teachers about the lessons that I would miss and the possibility of sending new topics to my email. The lessons will be complicated without thorough preparation. I have attached application form below this message for your review.
