Dear Sir / Madam,

I am writing to formally report the loss of my student ID card and to request a replacement. My ID card number is 23451126.

Unfortunately, I have been unable to locate it.As a result, I am unable to access university facilities such as library and other campus facilities which has caused considerable inconvenience for me.Not only this, I have difficulty entering some courses and exams without an ID card.Therefore, This situation negatively affects my academic studies, as it prevents me from fully participating in classes, accessing essential resources, and attending exams without difficulty.

Consequently, I request that a new student identification card be issued to me at your earliest convenience. If there is anything I need to do for the necessary procedures, please inform me.

Thank you for your attention, I look forward to hearing from you as soon as possible.
