I am writing to inform you about the incident that happened in the supermarket under your supervision.
My name is Sarina Shrestha, I am a local resident in the area. I am 24 years old and live in the 126 dunlop Street. I work as a social worker in the deafblind ontario services.
On february February 14th me and my husband were in the supermarket to celebrate the valentine’s day. I was involved in a minor accident inside the supermarket during that time. I had a fall injury in the washroom of second floor near the japaness restaurant. The washroom was just mopped but did not have any sign boards or notice on the door stating the condition. Unfortunately, as soon as I enter the washroom I sliped Slipped on the wet floor and sprained my ankle ankel. There were no employees on sight and no help was available. I had to call my husband to get me and drove to nearest clinic for first aid.
I understand that sometimes we all make mistakes, and our memory fails us, but accident like this could have been worse if an elderly was involved. Moreover, there was no notice and fall prevention sign boards on sight. This is a major issue and a sign of negligence from the employee. I suggest that you do a safety inspection and provide a first aid and precaution training for all the employees to prevent such occurrence in future.
