Dear Mr. Jones,

I am writing to you to inform you that I need to take an urgent leave. Unfortunately, an emergency happened due to my mother’s medical condition. It has worsened, and the doctor strongly advised that she must undergo an operation, and any delays would not be in her best interest. Furthermore, I am the only available person to take care of my mother since my brother is studying in another city.

I would like to take a two-week leave because the surgery is scheduled 3 days from now. Since I have accrued some leave from unused leaves from past years, I would like to use it now. Thus, the exact date of my leave starts on the 5th of May until the 19th of May. I will be able to start work on the following day.

Rest assured that before my leave, I will teach and train Miss White and Miss Black to do my work properly. Additionally, I will provide them with details that they will need. Moreover, they can call me during my leave should anything urgent arise.

I hope you will take my case into consideration and approve my leave.
