Dear Sir or Madam

I am writing this letter to inform you that I lost my valuable item while I went holiday last week.

I was lost my smartphone, I bought six months ago. The characteristic smartphone is the color is black with the blue case. I putted accessories in behind of the case and also my picture.

The accident happened when I went to the bathroom. I totally remember I brought my smartphone. I putted it in the bathroom while I was washing my hand. I forgot to take it. In a few minutes I realized that my smartphone leaved in the bathroom. So, I went back and I did not find it.

I have bought travel insurance to cover all of the item I have been lost when I enjoyed my holiday. So, I want to claim my insurance about losing my smartphone. I hope you can inform me about the rules I need to following.
