Dear Sir or Madam

I am wiriting this letter to extend my sincere thanks to Mrs Baker, the nurse of your hospital.

I am a teacher and every day i take a taxi to arrive my office. Last Monday morning when bus was coming and almost reached the stop. The bus and the taxi which i got in suddenly crashed each other. I felt dizzy and lost my consciousness. When I came to, I found myself in St. Mary’s hospital, attended my dad. The doctor told me it was Mrs Baker who looked after me and sent me to the hospital. She was in a bus station and watched the accident.

I and my family were very much grateful to Mrs Baker for her kindness. Please forward my heartfelt thankfulness to her. We invite her to our house for dinner.

I do hope that your company would reward Mrs Baker for her noble deeds.
