Dear Sirs,
I am writing to you to express my worries about an urgent matter. I am really sorry to inform you that I had a meal in your restaurant, the New Moon, two days ago on Monday at 9.00 p.m. After the meal, I felt ill and I had to go to the National Hospital in Third Avenue. Furthermore, I had a stomachache, and I felt nauseous. Consequently, my suffering lasted for more than six hours.
For your information, I had a plate of mushroom ravioli and pasta with chicken sauce. I am extremely concerned that the food was spoiled or had some harmful ingredients. It was disappointing because I usually eat in your famous restaurant, and I have never had such a miserable accident.
Now I am worried that someone else may have similar symptoms or get hurt. I would be grateful if you could check that everything is OK. In addition, it will be useful if you send a sample of the mentioned plates for investigation. Moreover, I would like to request a full refund for my $300 meal due to my resulting health issues. A photo of the receipt of the meal and the hospital report is attached. Overall, I am looking forward to your generous reply within ten days.
Yours sincerely,
Dina
