Dear Mr Smith,

I am writing to kindly request about allow for me. I would greatly appreciate for your attention in this matter.

The reason I am not feeling well. Because I have serious illness. That’s why I am calling you, yesterday I go back school to home. Then I passed out. Actually I have been feeling sick for three weeks, this is terrible on me. Doctor said me you should be taking medicine. So, I need to take medicine during two weeks.

I want to continue my studies online platform. I have some information about this. Addition I asked a permission to stay at home. If I have staying at home I can control study online.
