Dear sir/Madam,

It’s my please for getting an community award, I’m really happy and wants to thank the community for selecting me. I was really excited to receive the present unfortunately, I had an road accident and got my leg fractured and will not be able to attend the ceremony.

If community members agree, we arrange an alternative like I can send my family member to the ceremony on behalf of me also will write a note to thank everyone and apologize to be absent on this big day.
