Dear Mr. Lawrance,
I am writing to inform you that I had a minor accident in the store room during my shift on Friday and injured my back.
I worked the afternoon shift on Friday and was clearing some stocks in the store room, while it was a bit quiet in the shop. Two big boxes of coffee beans were delivered along with the logistics delivery and there was no space on the bottom shelves to store them. Hence, I had to use the step ladder to place them on a higher shelf and hurt my back. I felt a shooting pain at the time, but it got better the next day.
I went to the doctor the next day to make sure that it wasn’t anything major and was advised to rest for at least a couple of days. Unfortunately, I will not be able to work for the next couple of days and will provide you with a medical certificate once I’m back next week.
In order to avoid such incidents from happening in the future, I was hoping we could have a designated shelf for large boxes such as coffee beans and other products, so it’s more convenient for all team members.
Please let me know if you need any further information about the accident or any other documents to be provided to the Human Resources team.
