Respected Sir,

I am Lakhvir Singh, student of Loyalist College, North York. I am enrolled in 3rd term of supply chain management course and my enrollment id is 13553. I am writing this letter to you to talk regarding my project that I could not able to submit before deadline.

The deadline of project was last week but due to my poor health condition, I could not submit it. In the starting of last week, I went to attend a family function with my parent where I tried some thai food for first time that was looking very delicious at first sight. However, after some time I started having pain in my stomach area. I thought it might happened just because of fried food but when I reached home , I started feeling nausea and vomiting. Then, my parents took me to hospital and doctor told that I had a food poisoning. That time, I stayed three days in hospital and could not able complete my assignment.

I have attached my medical note with this letter for your reference. I want to request you to allot me three more days to submit my project so that I would not loose grades in subject. I shall be very thankful to you.
