Dear Sir or Madam

I would like to apologise for my early leave and problems it may caused. In order to solve this issue, I would like to reveal the reason for my departure and discuss about how I can compensate for missing the session.

Firstly, The main reason for my actions was my health condition. A day before I had to leaved the warkplace, because when I started working, I started to feel dizzy and a stomach ache appeared. All the documents, that could be approved as evidences, are linked to this letter.

I fully understand the consequences for workshop, caused by my actions. The sudden abandonment of workplace left a significant part of work undone. As a compensation, I would like to complete other tasks or increase the workload, If it is possible.
