Dear Sir/Madam,

I am writing regarding a request to get some time off work. Recently, my mom has become ill and she needs brain surgery. I am her only child and there is no one to take care of her but me, so I need to leave work temporarily to be by her side.

The recovery time of the surgery is two weeks but I need an additional week to make sure she can maintain her basic functions. This means that I need three weeks leave.

Regarding my responsibilities, I can maintain simple tasks while looking after my mother, such as reading reports and analyzing them. For harder tasks like handling meetings with vendors and implementing marketing campaigns, my co-worker, Bria, can effectively replace me. Additionally, if I am needed for any unexpected situation, feel free to contact me.

I would appreciate it if you would consider my request and I hope to hear from you soon.
